Job Description: This position provides expert, high-level knowledge of health care, technical skills and analytical expertise in order to aid health care professionals and direct internal resources as they improve health care along the continuum of patient services. The BMT Coordinator is responsible for navigating patients through the multiple testing, consultations and/or treatments that patients receive in the BMT/Acute Leukemia Program or in conjunction with referring oncology practices. Through ongoing and extensive interaction with the patient, BMT Coordinators determine the most appropriate strategies and logistics to execute the plan of care that is initiated and outlined by the attending physician. BMT Coordinators are responsible for understanding and applying Treatment Plans, SOP's and disease specific criteria to each patient, in order to assist the patient in moving through the transplant or Acute Leukemia process.Scope BMT coordinators follow multiple patients at any given time who are active in the pre-transplant process as well as following acute leukemia patients through their multiple courses of chemotherapy. Additionally, the coordinators are responsible for patients in the post-transplant/post-leukemic treatment process. This individual works closely with the BMT/Leukemia physicians and additional team members, such as midlevel providers, nursing staff (inpatient and outpatient), pharmacy and other associated with the patient's plan of care. The BMT coordinator must have exceptional communication skills with all members of the BMT/acute leukemia team. Job Essentials Patient Related Activities: Acts as the primary point of contact for the BMT / Acute Leukemia Program patients. BMT Coordinators are responsible for introducing the program, arranging referrals to appropriate clinical providers and answering preliminary, non clinical questions. Acts as the primary liaison between physician, patient and others associated with the patient's care in the BMT/leukemia program. Facilitates execution of care outlined by physician or other medical provider. This includes facilitating completion of outlined procedures and arranging for ordered testing or consultations in the appropriate timeline as deemed by the attending physician. Under the oversight of the BMT attending physician, BMT Coordinators manage the logistics of a patient's on-going post-transplant care, including evaluations at certain milestones, varied procedures/testing/results and discussions of progress. This ongoing relationship can continue for years or the lifetime of each patient. Actively facilitates and presents patient caseload in weekly tumor board to discuss status of patients in the program, provide updates regarding course of treatment, ensure completeness of medical record, alert medical providers of pertinent issues that could affect course of treatment (co-morbidities, issues regarding current treatment, caregivers/family issues, lack of housing if out of state or lives far from the hospital, insurance denials etc). Collects all pertinent information from the patient and 'source documents' from their referring physician. This may include medical history/background, treatment/chemotherapy history, laboratory results and co-morbidities that might influence transplant care, demographic information for matriculation into program and hospital, insurance and financial information. Coordinate and perform the responsibilities related to the BMT/leukemia patient including scheduling visits, screening needs, accompanying the BMT/Leuk attending in the patient consent process. If the patient needs a matched donor for transplant, the BMT coordinator has oversight of the procurement process. This would include collecting pertinent information from family donors and/or volunteer donors and the process of arranging for appropriate lab testing locally or nationally for specimen collection. Donors are identified by the BMT coordinators through the HLA typing process. The BMT unrelated search coordinator independently is able to choose appropriate unrelated donors for HLA typing. In addition, recommendations are made by the coordinator to the HLA lab for further testing based on the patient's antibody or HLA results. Participates with the physician and mid-level provider during the initial assessment as they report and/or discuss the findings from their review of the patient's history and results of the physical exam. This allows the coordinator to understand and assist in implementing the recommendations and care plan designed by the physician. Assess patient funding sources and determine whether patients have coverage adequate to proceed to the next step in treatment. If funding is insufficient, the BMT Coordinator is responsible for researching appropriate funding sources and referring the patient to appropriate community services. BMT coordinators also manage the logistics of appeals processes with a patient's insurance company, working with insurance case managers and medical directors to escalate appeals for necessary procedures. Assists in ongoing patient education throughout the treatment process. Program and Protocol Related Activities: Actively participates in marketing the program, community awareness and education, and supportive activities to increase volume and enhance patient satisfaction. This activity includes meeting with referring physician's office staff during outreach visits. Understands, educates, supports and acts as a liaison to referring physicians. Contributes to program databases regarding protocol and patient data. Manage and report patient data required by the Center for International Bone Marrow Transplant Research (CIBMTR) and other national reporting agencies, as necessary. This process includes specific disease forms, collection and donor data as well as outcome and followup data. Maintains knowledge and reads and understands all BMT/leukemia program related treatment/research protocols and policies and procedures. Other Activities: Coordinates with The National Marrow Donor Program (NMDP) to assist in arranging for bone marrow donations as an NMDP collection center. Attends all clinical and basic BMT/Leukemia presentations (as applicable) such that theirknowledge of these advances might remain current. Assists and participates in BMT program accreditation surveys and inspections. Anticipates and recognizes issues and is proactive in problem resolution. Minimum Qualifications Successful completion of a Bachelor's Degree. 3 years experience working in one of the following settings: transplant, patient coordination or a position that requires ongoing and direct interaction with patients in a clinical setting. and - Highly motivated with a positive attitude and high degree of flexibility in a rapidly changing environment. and - Proven ability to work closely with, and maintain excellent rapport both in-house and outside hospital staff, administrative, multidisciplinary team, department managers and physicians to ensure quality patient care. and - Excellent communication and presentation skills equired and - Excellent interpersonal skills to foster collaborative partnerships using a team approach to span traditional boundaries within a health care setting. and - Strong analytical, writing, and critical thinking skills. and - Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications. Preferred Qualifications Familiarity with terms and processes in the field of autologous and allogeneic marrow/blood transplant. and - Familiarity with Federal Regulations for bone marrow transplant and donor processes. and - Experience managing research and data collection projects. Physical Requirements: Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs. and - Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. and - Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc. and - Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.
