Rutland UPCC Is Seeking a Family Physician To Provide Primary Care Services Within Its Longitudinal Care Clinic. This Interdisciplinary Clinic Is Comprised Of On-site Physicians, NP's, RN's And Social Workers Who Utilize a Team-based Care Model. The Patients Who Are Attached To This Clinic Have Primary Care Needs That Include But Are Not Limited To

• Chronic disease management
• Substance use disorders
• HIV primary care
• Gender affirming care
• Refugee care
• Full spectrum family practice

This is a unique opportunity for an incoming physician to take over a small, stable pre-existing patient panel as well as grow the panel size as they see fit, with support from an incredible allied health team.
