Transition Coordinator Overview

The role entails support for high school students, particularly those with intellectual disabilities and/or autism, in their transition to employment and post-secondary opportunities. The ideal candidate will have a bachelors degree in a related field (Masters preferred) and at least two years of experience working with high school students, preferably in college counseling or vocational coaching.

This role involves supervising staff, developing job training opportunities, monitoring student progress, coaching at employment sites, and facilitating connections with businesses and community agencies. Strong communication, organizational, and data analysis skills are essential. Travel across Long Island is required, and a valid drivers license is necessary.

Education & Experience

Education: Bachelor's degree required in Psychology, Guidance Counseling, Social Work, Special Education, or a related field; masters degree preferred.

Work experience: Minimum of 2 years working with high school students, preferably those with intellectual disabilities and/or Autism. Prior experience in college counseling and/or vocational coaching is strongly preferred.

Knowledge, Skills, & Abilities

Strong oral and written communication skills.

Excellent time management, organization, and prioritization skills.

Proficiency in computer programs such as email, word processing, spreadsheets, databases, and web-based platforms.

Familiar with record-keeping, data collection/analysis, summarization, and graphing.

Ability to build and maintain collaborative working relationships with stakeholders and facilitate group discussions to reach consensus.

Knowledge of ACCES-VR and other NYS services, including OPWDD and OMH.

Must have a valid drivers license and be able to travel throughout Long Island.

Essential Duties & Responsibilities

The following statements are intended to describe the general nature and level of work being performed by individuals assigned to this position. Supervision (~60% of role):

Supervise field staff to ensure adherence to CEO standards and best practices.

Assist in the management of relationships with partner districts.

Attend professional development training on regulatory updates and procedural changes.

Ensure timely and accurate submission of student schedules, reports, and activities.

Participate in recurring meetings regarding service implementation and operations.

Monitor students at job sites alongside Career Consultants.

Develop knowledge of employment opportunities and training programs for placements.

Gather and analyze student background information to match them with suitable job and post-secondary opportunities.

Conduct Job Analysis Surveys with employers before student placement.

Oversee data collection on student progress using Task Analysis and Industry Standard.

Ensure field staff properly enter inventories, assessments, and reports into CEO systems.

Assist Career Consultants with student behavior management using positive support.

Support field staff with clerical and instructional preparation tasks.

Attend CSE meetings when requested.

Perform other similar duties as needed.

Coverage for absent consultants & other direct provision of service (~25% of role):

Offer job coaching and on-the-job training, assisting employers with student orientation and workplace integration.

Collect and record information on student performance through progress monitoring using Task Analysis and Industry Standard data.

Assist with student behavior management using positive behavior supports.

Handle clerical and instructional preparation tasks, including setting up materials.

Identify student strengths, job accommodations, and solutions to employment barriers.

Guide students in exploring post-secondary education, careers, and independent living options, connecting them to community resources.

Provides individual and group work-readiness skills training to students in districts and through ACCES-VR in areas including resume development, interview skills, networking, employer expectations, independent living skills, positive work attitudes, developing initiative, motivation, and responsibility.

Perform and enter all appropriate inventories, assessments, and reports pertaining to the students transition or ACCES-VR client into company systems.

Perform other similar duties as needed.

Job Development (~15% of role):

  • Utilization of the CEO strategies for job development.
  • Ongoing job development for placing students/clients into internships, jobs, or for job exploration, and/or mentor programs.
  • Work with Career Consultants and team to develop marketing plans for student/client needs and district program requirements.
